Applicants should submit the following requirements on or before OCTOBER 10, 2016:
1. A letter stating the position applied for;
2. Duly accomplished Personal Data Sheet (CSC Form 212);
3.*Transcript of Records;
4.*Certificate of Eligibility;
5.*Certificate of Seminars/Trainings Attended;
6.*Service Records or Certificates from previous/present employers stating the positions assumed and corresponding actual duties and responsibilities (if applicable);
7.*Certificate of no pending case (if applicable); and,
8.*Performance Appraisal Reports for the last two rating periods (for current government employees only);

*Where photocopies are filed, the documents must be certified as true copies by the proper authorities or the applicant may present the originals to the Human Resource Management Officer/Administrative Officer IV for comparison with the photocopies.

APPLICATIONS WITH INCOMPLETE DOCUMENTS AND NOT RECEIVED ON OR BEFORE THE DEADLINE SHALL NOT BE INCLUDED IN THE SCREENING PROCESS.
